HIPAA Notice of Privacy Practices · Effective February 16, 2026
This Notice of Privacy Practices describes how MDRN Dental Studio may use and disclose your protected health information (PHI) to carry out treatment, payment, and healthcare operations, and for other purposes permitted or required by law. It also describes your rights to access and control your health information.
We are required by law to maintain the privacy of your health information, to provide you with this notice, and to follow the terms of the notice currently in effect. We reserve the right to change our privacy practices and the terms of this notice at any time, as permitted by law. Changes will be posted at our office and available upon request.
How We Use and Disclose Your Health Information
Treatment
We may use and share your health information to provide, coordinate, or manage your dental care and related services. This includes sharing information with specialists or other providers involved in your care.
Payment
We may use and share your health information for billing, insurance claims, collections, coverage determinations, and reimbursement from your insurance company or other third parties.
Healthcare Operations
We may use and share your health information for quality assessments, training programs, and activities necessary to run our practice and ensure you receive quality care.
Individuals Involved in Your Care or Payment
We may share your health information with family members, friends, or representatives you identify as involved in your care or payment, unless you object.
Required by Law
We will disclose your health information when required to do so by federal, state, or local law.
Public Health Activities
We may share your information as required for public health purposes, including to:
- Prevent or control disease, injury, or disability
- Report child abuse or neglect
- Report reactions to medications or problems with medical devices
- Notify individuals of recalls affecting their care
- Report suspected abuse, neglect, or domestic violence to authorities

Fundraising
We may contact you about sponsored activities or fundraising programs. You have the right to opt out of receiving such communications at any time.
Secretary of HHS
We will disclose your health information to the U.S. Department of Health and Human Services when required for HIPAA compliance investigations.
Substance Use Disorder Treatment Information
If your information comes from a program covered by 42 CFR Part 2, it will only be used or disclosed as expressly authorized by your consent, or as otherwise permitted by law. We will not use or disclose this information in any civil, criminal, administrative, or legislative proceeding without a court order or your written authorization.
Other Uses and Disclosures
We will obtain your written authorization before using or disclosing your health information for purposes not covered in this notice or otherwise permitted by law — including disclosing psychotherapy notes, using your information for marketing purposes, or selling your information. You may revoke any authorization in writing at any time. We will stop using or disclosing your information upon receipt of the revocation, except for actions already taken in reliance on it.
SMS Text Messaging
When you provide your phone number to MDRN Dental Studio and consent to receive text messages, we use your number to send appointment reminders, appointment confirmations, care follow-ups, customer service responses, and other communications related to your dental care.
Information Collected
We collect your name, phone number, and any information you share when responding to our messages.
How We Use This Information
We use your information solely to communicate with you about your dental care and our services. We do not sell or share your mobile number or SMS opt-in consent with third parties or affiliates for marketing purposes.
How to Opt Out
You may opt out of SMS messages at any time by replying STOP to any text message from us. For help, reply HELP. Message and data rates may apply. Message frequency may vary.

Website and Digital Services
This section describes how we collect and use data through our website and digital tools — separate from your clinical health information.
Online Scheduling
Appointment requests submitted through our website are processed by our scheduling platform. Information you enter (name, contact details, appointment preferences) is used solely to confirm and manage your appointment.
Google Reviews
Our website displays patient reviews sourced from the Google Places API. When you visit pages that display reviews, your browser may make requests to Google's servers subject to Google's Privacy Policy. We do not collect or store data from these requests on our end.
No Analytics Tracking
We do not currently use third-party analytics trackers (such as Google Analytics or Meta Pixel) on this website. We do not sell, rent, or share your website browsing data with advertisers.
Contact Information
If you contact us via the website contact form, email, or phone, we retain your contact details only to respond to your inquiry and schedule care. This information is not shared with third parties for marketing purposes.
Your Health Information Rights
Right to Access
You have the right to inspect or obtain a copy of your health information, with limited exceptions. Requests must be submitted in writing. We provide electronic copies of records maintained electronically, and paper copies of paper records, in the format you request if readily producible. Reasonable cost-based fees may apply.
Right to Amend
You may request that we amend your health information if you believe it is incorrect or incomplete. Requests must be submitted in writing with a reason for the amendment. We may deny requests under certain circumstances and will provide written explanation if we do.
Right to an Accounting of Disclosures
You may request a list of certain disclosures we have made of your health information. Requests must be submitted in writing. Additional requests within a 12-month period may incur a reasonable fee.
Right to Request Restrictions
You may request restrictions on how we use or disclose your health information. We are not required to agree to a restriction except when the disclosure is to a health plan for payment or operations and involves a service you paid for entirely out of pocket.
Right to Confidential Communications
You may request that we contact you by alternative means or at an alternative location. We will accommodate all reasonable requests. Please specify the alternative contact method in your written request.
Right to Breach Notification
You will receive timely notification if there is a breach of your unsecured protected health information, as required by law.
Right to a Paper Copy
You may request a paper copy of this notice at any time, even if you have agreed to receive it electronically.

If you have questions about this notice or believe your privacy rights have been violated, you may contact us directly or file a written complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
